Perthes diseases
It is a hip condition.
It is characterized by avascular necrosis in case of supracondylar fracture and medial condylar fracture.
Causes-
Fracture of neck of femur
Avascular necrosis
Children ( 3-6 years ) - common
Boys>girls
Unilateral, may be bilateral in some case
Low birth weight and high birth weight
Clinical features-
Pain in hip region
Pain knee and hip
Avascular necrosis
Tenderness
Decrease ROM
Internal rotation and abduction decreases
Affected side pelvic drop
Scoliosis
Antalgic gait
Trendelenburg gait ( in bilateral perthes disease)
Diagnosis
Physical examination
X-ray
CT scan( soft tissue)
MRI( ligament, tendon)
Treatment
NSAIDS, steroids, or analgesics.
Arthrotic devices using splint and braces
Surgical
Femur osteotomy
Arthroplasty
Arthrosynthesis
Arthrodesis
